One challenge that nurses often face is patient resistance to established goals. This can be a result of patients’ fears and concerns about their health or lack of understanding of the education they receive from nurses.

Outcome

The nurse’s process of assessment involves collecting, organizing, validating and documenting data regarding a client’s health status. This includes physiological, psychological, sociocultural, spiritual, economic and life-style data.

The outcome of this process is a nursing diagnosis that the nurse then implements into a care plan. This care plan is the framework for setting short-term and long-term goals.

The nursing care plan is used to measure a client’s progress towards the goals of the nurse’s assessment. The client’s response to the nursing interventions determines whether the desired outcome was met, partially met, or not met.
